Analysis of the current situation and influencing factors of the post-discharge coping difficulties in women with hypertensive disorders of pregnancy

Objective To describe the status of the post-discharge coping difficulties in women with hypertensive disorders of pregnancy(HDP),and to explore its influencing factors. Methods A total of 280 women with HDP from a tertiary A maternity hospital in Nanjing City were investigated with the general information questionnaire,the post-discharge coping difficulty scale-new mother form,the readiness for hospital discharge study-new mother form,the family APGAR index. The factors influencing post-discharge coping difficulties in women with HDP were analyzed using univariate analysis and multiple linear regression. Results 238 valid questionnaires were collected,with a response rate of 85.0%. The score of the post-discharge coping difficulties in women with HDP was 5.07±1.69,which is at a middle level. The results of multiple linear regression analysis showed that parity,primary caregiver during the postpartum period,HDP type,readiness for hospital discharge,family APGAR index were influencing factors of post-discharge coping difficulties in women with HDP(P<0.05). Conclusion Post-discharge coping difficulties in women with HDP remains to be further improved,and it is affected by many factors. Medical staff should pay more attention to post-discharge coping difficulties of such parturients,formulate targeted continuation care programs according to relevant influencing factors,comprehensively improve their coping ability,and reduce their coping difficulties after discharge.

The relationship between task load, stress cognition and psychological disengagement among midwives in Jiangsu

Objective To investigate the current situation of psychological detachment(PD) of midwives in Jiangsu,and to analyze its influencing factors,with the aim of providing a theoretical basis for managers to take appropriate interventions to improve the level of PD of midwives. Methods Using a convenience sampling method,614 midwives from 13 cities and 42 hospitals of different levels in Jiangsu from August 2022 to September 2022 were investigated by this cross-sectional survey. A self-developed questionnaire,Psychological Detachment Scale,the Traumatic Stress Scale for Midwives,the Jefferson Empathy Scale and National Aeronautics and Space Administration-Task Load Index were adopted to collect data. Results A total of 590 valid questionnaires were collected. The mean total score of the Psychological Detachment Scale was(9.24±3.45). The marital status,quality of sleep,frequency of after-work use of electronic devices for work,traumatic stress,empathy,and task load were the influencing factors of PD(P<0.05). Conclusion The low level of PD of midwives is influenced by a variety of factors. Managers,therefore,need to attach importance to the PD of midwives and formulate targeted interventions to improve the level of PD and work recovery experience with a view to promote their work engagement and quality of care.

A qualitative study on the experience of farewell ceremony based on perinatal care for women with perinatal loss of fetus

Objective To deeply understand the inner feelings of farewell ceremony on pregnant women with perinatal loss of fetus. Methods A descriptive qualitative approach was conducted and a total of 15 women experiencing perinatal loss were enrolled for semi-structural in-depth interviews between March 2020 to March 2021 from a tertiary A obstetrics and gynecology hospital in Nanjing. The Colaizzi method was used to analyze the data. Results 3 themes were extracted,namely needs of farewell ceremony,the interweaving of complex emotions,change of perceived social support. Conclusion The farewell ceremony based on hospice care can help mothers reasonably release their negative emotions of perinatal loss,help them relieve stress,anxiety,ease the pain of loss,reduce the occurrence of the depression. Nursing staff should pay attention to perinatal maternal psychological needs,and help them face and accept the reality of the loss with professional technology and means,and ultimately realize the healthy development of family.

Development of review indicators and analysis of obstacles for oral intake management during labor

Objective To implement evidence-based practice of oral intake management during labor,develop review indicators,analyze obstacles and facilitators during implementation,and formulate reform strategies. Methods With the Joanna Briggs Institute evidence-based health care model as the guiding framework,an evidence-based practice team was formed after the clinical nursing problem was identified to search,evaluate and summarize the evidence,and to develop review indicators as well as review methods. Obstacles and facilitators were analyzed according to the baseline review results,and reform strategies were formulated. Results Totally 18 pieces of best evidence were included and 13 review indicators were formulated. There were 9 indicators with accurate execution rate of <60% and 5 of them were 0. The main obstacles were the lack of attention and relevant knowledge of midwives,the lack of assessment tools for risk factors of restricting oral intake during labor,specific dietary recommendations,management procedures as well as educational materials for different groups. The main facilitators were strong organizational leadership,multi-disciplinary team support,and high maternal and family participation and etc. Accordingly,corresponding reform strategies were formulated. Conclusion The review indicators constructed based on best evidence and combined with professional judgment were scientific,effective,appropriate and feasible. The analysis of obstacles and facilitators and the formulation of reform strategies can provide a guarantee for promoting the clinical practice of oral intake management during labor.

Men’s lived experiences of partner perinatal loss:a meta-synthesis of qualitative research

Objective To synthesize the findings of qualitative research exploring men’s experiences of partner perinatal loss,and to provide a basis for constructing psychological interventions. Methods Literature retrieval of qualitative research on men’s experiences of partner perinatal loss was conducted in 8 databases,including SinoMed,CNKI,Web of Science,Pubmed,CINAHL,psychINFO,embase,Cochrane library,from the establishment of the databases to April 30,2021. The JBI Critical Appraisal Tool for qualitative studies from Australia was used to evaluate the methodology quality of the included research,and the meta-aggregation was used to conduct the synthesis. Results 12 qualified studies were included and integrated into 59 valuable findings. These findings were then classified into 11 categories under 5 groups,namely men’s complex emotional experience after perinatal loss,the negative changes in life,the different coping strategies,difficulties and needs,and the discovery of benefits. Conclusion Nurses should break up the stereotypes about males,pay more attention to their emotional experience in perinatal loss,and cooperate with their families to provide them with information,environment and humanistic support under the guidance of hospitals. In this way,it will be easier to reduce the males’ care burden and the negative impact of partner perinatal loss on their life,so as to promote their personal growth.
